A Case of Membranous Obstruction of the 4th Ventricle due to Cyrpococcal Meningitis



Abstract
We have experienced a case of membranous obstruction of the 4th ventricle due to cryptococcal meningitis which was confirmed by the surgery and india ink preparation of the cerebrospinal fluid.
This 57 years old man was admitted to the Department of Neurosurgery, Catholic Medical Center in January, 1974 with chief complaints of headache, vomiting, fever and mental deterioration for 1 month.
On admission, neurological examination disclosed mental confusion, weakness of right lateral rectus muscle, horizontal" nystagmus, right supranuclear type facial weakness and incoordination on the right side.
Conray ventriculogra:z through the right coronary suture revealed markedly dilated 4th ventricle with obstruction of the ventricle.
Cryptococcus neoformans was discovered by india ink preparation of the cerebrospinal fluid.
Membranous obstruction of the foramen magendie was found through the postereior fossa exploration. The biopsy specimen of the membrane disclosed fibrous membrane with infiltration of the inflammatory cells.
